- Please make sure you upload or take to school site upon request the following documents to complete registration. No child should be prevented from enrolling in school because he or she lacks a birth certificate or passport, or has one from a foreign country. Vaccines
-
Current address verification
-
Recent physical exam may be required (depending on student’s grade level)

-
If your student is coming from another school district, you will need to provide the previous school name and/or district information so the school can request your student’s educational cumulative file.
-
If your student has an IEP or 504 plan, alert the staff at the time of registration so the additional supports or modifications can be provided for the student.
-
If you are working with a student in transition (multiple families living under one roof) or a foster child, you may be entitled to receive additional supports and services. Please contact Healthy Start to review your eligibility for these programs.
-
If you do not want to attend your school of attendance, please complete an Intra-District Transfer 2024-2025 intra district.
-
Please complete the Inter District Transfer Form if you would like to attend a school outside of PVUSD and return to the Student Services Department (294 Green Valley Rd., 3rd floor). You can also email the completed form to: idt@pvusd.net
